Truck Driving Program Requirements

Students must bring the REQUIRED documentation with them the first day of class:

• Driver License
• Social Security Card
• 3-year DMV printout of driving history (Must be DIRECTLY from DMV no online documents will be accepted)
• High School Diploma / Transcripts, GED or college diploma/ transcripts
• Permit
• DOT Medical

*YOU WILL BE REQUIRED TO TAKE A DRUG TEST THE FIRST
DAY OF SCHOOL ONCE YOU’VE COMPLETED ORENTATION*

• Valid/ Current Passport OR Certified copy of your Birth Certificate
• Social Security Card
• Driver License
• DOT Medical Report

• Mortgage Bill
• Home Utility Bill
• Employment Document
• Tax return
• Car Registration
• Phone Bill
• Bank Statements
